Forms

Nutrition First will provide all the necessary forms for participating in the program, along with access to KidKare. This platform allows you to enter meal details, meal counts, and attendance for children.

Program representatives will deliver enrollment forms during each visit. If you run low on forms, you can call the office to request additional copies by mail, or you can print them directly from this page.

Keeping Records

USDA requires all providers to maintain copies of all submitted forms for reimbursement, as well as corresponding documentation, on-site for a minimum of three years, plus the current year.
